Sexual stimulation releases nitric oxide, which raises cGMP levels and relaxes smooth muscle in penile blood vessels. That relaxation lets more blood enter erectile tissue.
Sildenafil inhibits PDE5, the enzyme that normally breaks down cGMP, so the signal lasts longer. It supports an erection only after stimulation has started that pathway; it does not create one on its own.
